Job Title: Community Health Evaluation and Data Specialist

Purpose: Supports community health team in tracking data and outcomes, data input, and database training as well as developing reports and tools to assess impact.

Reports to: Evaluation and Data Director 

Accountable to: Director of Community Health, Assistant Director of Community Health, Health Supervisors, Enlace leadership team, Evaluation and Data team

Communicates with: Community health team members, community health partners and funders

Informs: Enlace staff

Status: Full-time
